Where Birth Certificates are Filed

Although the birth of a child is no longer directly recorded at the municipal office since August 2013, certified copies of birth certificates can be obtained through the clerk’s office of where the child was born or where the mother lived when the child was born.

Required Identification and Proof of Interest

To request a certified copy, you must provide proof of identification and demonstrate a direct and legitimate interest if requesting a record besides your own. Acceptable forms of identification include:

  • Driver’s License
  • Passport
  • Government-issued ID
  • Or two forms of secondary ID, such as a utility bill, bank statement, or a previously issued vital record.

How to Request a Certified Copy

To request a certified birth certificate, please send a written or faxed request to the Town Clerk with the following information:

  • Date of Birth
  • Full name at birth, including first, middle, and last name
  • Your mailing address
  • The number of certified copies desired
  • Your relationship to the individual on the birth certificate
  • Photo ID and documentation proving direct and legitimate interest
  • If authorizing someone else to obtain the certificate, complete an Authorization to Release a Vital Record Form
