Updated Regulations for Vital Records
As of July 12, 2010, state law requires that individuals requesting certified copies of death records must provide identification and evidence of a direct and legitimate interest if they are not the spouse of the deceased.
Identification for Records Request
Acceptable forms of identification include:
- Driver’s License
- Passport
- Government-issued ID
- Or two secondary forms of ID, such as a utility bill, bank statement, or voter registration card.
Requesting a Certified Copy
To request a certified death certificate, please send a written or faxed request to the Town Clerk including:
- Date of death
- Full name of the deceased (first, middle, last)
- Your mailing address
- Number of certified copies requested
- Your relationship to the deceased
- Photo ID and supporting documentation for legitimate interest
- If authorizing a third party, an Authorization to Release a Vital Record Form is required

- Additional Copies: $6 each (when ordered at the same time)
